One testing kit is required per animal.  Each testing kit contains one of the following: Sample Collection Card and 18-gauge Needle (if required).

Sample Submission forms and Instruction sheets can be downloaded and printed from our website.  If you would like a form sent to you, please check the appropriate box below.
